Thinkpad W500 video to external monitor fails after reboot

Description
With Thinkpad W500, Panorama works beautifully at 1920 x 1200 on the LCD screen and also out to an external VGA-connected monitor.
However, if the Thinkpad is connected to the external monitor and I reboot eCS, then the display is now directed to the LCD screen instead of the external monitor.

On boot, the BIOS decides how it is going to configure the hardware. The Panorama software doesn't have anything to do with this.

Running ACPI 3.22.03 and Panorama 1.06, the Thinkpad now boots reliably to VGA if the laptop is plugged into the external monitor.
