 At a minimum, you must include the bldlevel output for the driver you are using.  Also, include the PCI Vendor ID and Device ID of your hardware in the ticket (ie. the section from PCI.EXE that relates to your NIC). If the driver traps, please, include the trap screen.

-The distribution packages include a debug build of the driver. The debug build should not be used unless you are debugging a problem and the developer has requested the debug output. The debug build is much larger and much slower than the retail build, and is specifically designed to produce debug output. The debug build is not designed for actual use and in many cases the debug build will not work properly or may not work at all.
